Warcraft 3, Human Campaign, the defense mission against Undead. I was a little kid at the time, and getting swarmed by undead was scary, particularly given my rather vivid imagination.

For me without question silent hill on the PS1. Other games had given me the cheap jump scares so overly used in modern horror movies, but silent hill was DEEPLY unsettling and truly scary throughout. The feeling of relief only coming when you reverted back to the normal overworld from the "hell side".
The soundtrack is a work of genious no doubt.

I'm not quite sure if this counts as being scared, but the first time that a game's ever made me feel sick to my stomach was when I saw the bad ending to Abe's Oddysee. It's not an over-the-top or grotesque cutscene, but it makes use of what you don't see and just hints at what happens to Abe which is the kind of thing that destroys me the most due to my overactive imagination. Granted, I saw it when I was pretty young, so maybe I wouldn't be as scarred by it today.

The earliest "game" that I can remember that scared me was the scary maze game. Not sure if it qualifies as a "game" though, since there really isn't a way to "win", you always get the screamer.
Otherwise like someone else mentioned, Star Wars: Shadows of the Empire for the N64. Specifically the IG-88 fight after the train crash; the cut-scene before was creepy, it chased you around making angry robot noises, and could jump really far. As a side note, the button combo to enter the debug mode was insane to enter with a controller. Hold all the C buttons, both shoulder buttons and Z, left on D pad, and then push the analogue stick half way and hold for a couple seconds to the left and right a few times until an audio cue would play. You had to do the analogue stick with your face.

Most likely Resident Evil 3, when Nemesis burst through the window.
A friend and I wanted to play the game in the dark, it messed us up for a week or so haha.
Most recently though it was probably Arkham Asylum, during a scarecrow scene and the "game" glitched, I panicked lol

Probably the original Silent Hill.
But not many games actually scare me, P.T on the PS4 is one of the rare ones that was genuinely scary, just wish Konami hadn't shit canned the full game.

A nice little sci-fy strategy game from 98 called Armor Command.
The general atmosphere and sublime soundtrack gave off a very sinister vibe, as well as the games Fog of War mechanics (units just fading in) made things very interesting. ^^
I never played anything else like that until Doom 3 and later Dead Space.
