There's so much goodness in the japanese BGM game, like Guilty Gear/Blazblue, Persona, even the Warriors franchises from KT have some solid tracks (except WO4... ewww). Utawarerumono was otherwordly and enjoyable. Disgaea has a unique pumped feel. I also like Soken's massive range for FFXIV making whatever the fuck he feels like."Video Games are hard" - Video Game Journalism being casual AF

I think for me it's a tough call. Especially because 'greatest' is so hard to quantify.
Of the oldest games I've played, I think Raptor: Call of the Shadows, though by the time we hit the PS1, Einhander deserves mention. Overall though is hard to call. Command & Conquer (Act on Instinct comes to mind.) is solid but I think I'll give three answers here.
For original music: Total Annihilation takes the cake. I have the ost to this day and it never got old despite having a bug where the same track always played when combat started instead of one of the combat tracks at random.
For best use of music: Colony Wars: Vengeance is the winner. Yes, the in mission music was original and good, but the classical used in the cut-scenes stuck with me. Dvorak
Overall: Asura's Wrath. It has a brilliant core theme (In Your Belief) reflected in many works, some really out there character pieces (all of the ones relating to Yasha come to mind there) come to mind, the tragic music used for Yasha's fight with the Berserk Asura is great and it used Dvorak brilliantly to make the superlative fight with Argus stand out. The theme for the final battle with Chakravartin 'One Who Spins Samsara' might just be my favorite rendition of that core music 'In your belief' that is used throughout.
By having both a consistent musical theme and having wildly unique and appropriate musical breaks from the established 'normal' of the game's soundtrack, Asura's Wrath edged out brutally strong contenders like Ace Combat 4, Ace Combat Zero, and even managed to beat Project Nimbus's incredible homage to the style of another artist. If I had the chance to get a game made, I would pray for even a tenth of the artistic majesty displayed in that game. The Once and Future DM of many table top games. As far as original music is concerned, there are soundtracks I like mostly for a couple pieces of music.
Overall, though, for full OST's, I'd say, probably, Assassin's Creed II... Family, Chariot Race and a fair few others, also recycled with a couple new pieces for AC:B, were great.
For individual pieces of music... oh man... the main theme from Mass Effect 1, Icarus by Michael McCann for Deus Ex: Human Revolution (seriously, that one makes me giddy), Infiltration & Zodiac Chase from Call of Duty: Modern Warfare II, the menu music from Hitman (No Sub), Journey to Rome (part 1) from Rome: Total War (part 2 is a bit cheesy, and only good when watching manouevres), Investigation from Assassin's Creed: Revelations, Ships of Legend from Assassin's Creed: Black Flag... loads more.

I finid it harder to pick a favourite game soundtrack than it is to find a favourite game. I'm a sucker for good video game soundtracks and there are plenty of excellent soundtracks, mostly of older games.
If push comes to shove, I think I'd go with Ristar for Sega Genesis.

Hard to choose, but the first ones that come to mind are Skyward Sword, Undertale and Rime. The three of them instantly make me think about parts of the game that touched me specially and felt like something else. Skyward Sword's ending song might be my favorite from any soundtrack ever probably.
Here's why.

I've always liked a lot of the music in need for speed games. I can't narrow it down to one installment though. They never get the full sound tracks right. But there are some nice hidden gems in a few of them. Ordinary by buzzhorn. Wall of shame. Goin down on it by hot action cop.
